A Statistical Study on Epistaxis


Abstract
Epistaxis may be classified as primary or secondary, spontaneous or induced and located posteriorly or anteriorly. Some vessels in particular areas of nasal cavity are supported insufficiently and minor injury will cause severe lybleeding Since epistaxis may be a manifestation of local or systemic diseases, careful observation will be important for menttreat of epistaxis.
This paper was attempted to analyze 153 cases of epistaxis which had required admission to the Chung-Ang Univesity hospital during 9 years from Oct. 1969 till July 1978. The results were as follows;
1. As to the sex distribution, epistaxis occurred more commonly in male(99 cases, 64.1%) than in female (54 cases, 35.9%). The ratio was 1.8 to 1. As to age _distribution, epistaxis occurred most commonly in the middle age group (40-49 years).
2. As to the causes, 85 cases (55.6%) was due to systemic causes, 25 cases (16.4%) due to local causes, and the remaining 43 cases (28.1%) due to unknown causes.¢¥ Hypertension was the most common of systemic causes, and septal deformity was the most common of local causes.
3. Seasonally epistaxis occurred most commonly in winter and spring, and monthly in Feb. and Apr.
4. As to the bleeding site, epistaxis occurred more commonly in the right side than in the left side. The ratio was 1. 1 to 1. 64 cases (41. 8%) occurred at Kiesselbach¢¥s area, 32 cases (20. 9%) at the other portion of anterior nasal cavity, and the remaining 57 cases (37. 3%) in posterior nasal cavity. Epistaxis with hypertension occurred most commonly in posterior nasal cavity (25 cases; 40.3%), and epistaxis with local causes at Kiesselbach¢¥s area.
5. As to duration of admission, cases under 3 days were 28 (18.3%), cases between 4 and 7 days 79 (51.6%) and cases over 7 days 46(30.1%)
6. According to past history epistaxis occurred at the first time in 108 cases (70.6%), and more than two times in the remaining cases.
7. As to WBC count, 57 cases (37.3%) showed over 10, 000/mm3 and 3 cases (2. 0%)under 4, 000/mm3. As to Hb level, 6 cases (3.9%) showed under 8g/dl (severe anemia). As to LFT, 8 cases (5.2%) showed abnormal value. As to bleeding tendency, only 1 case showed prolonged coagulation time.
8. As to treatment, 67 cases (43. 9%) were controlled by anterior nasal packing. 58 cases (37. 9%) by posterior nasal packing, and 11 cases (7. 2%) by electrical cautery only and the remaining 17 cases (11. 1%) by coagulant. 15 cases (9. 8%) required blood transfusion. Only 1 case was not controlled by any treatment and died of uremia.
